Please make sure you have completed all the requirements
for entry submission:
QUALIFICATION: To qualify for entry in the Awards Program competition, all rules and
regulations for entry submissions must be adhered to.
ELIGIBILITY: All publishers are eligible to compete. Entries must be from material
published and dated in the calendar year 2012.
JUDGING: Judging is conducted by a group of experts and Catholic press professionals.
Each entry will be reviewed by two different judges.
LABELS: Special labels are enclosed with this material. Photocopy the form if you need
additional labels. The labels are your entry’s identifier and must be affixed to the back of
each book. The entry will be disqualified if the label is not affixed to the book. Please type
or clearly write your labels, making sure to mark the award number with
the exact number and letter of award category. Award certificates are produced from these
labels. Please list the names of all contributing authors responsible for the entry.
SUBMITTING ENTRIES: Please read the description for each category and submit the
entry within the guidelines outlined in this packet. Please provide clean material with no
marks or highlighting on the submission. Wrap your entries carefully and insert the
purchase order and payment at the top of the materials. If you are sending it near the
deadline, be sure to use an express delivery service. Your purchase order and payment
must be enclosed with all submissions. Please do not send the purchase order and check
under separate mailing. To confirm receipt at the Catholic Press Association office, please
use your carrier’s online tracking service.
THREE COPIES: Please send three copies of each book per category.
SHIPPING: Please no individual boxes weighing more than 25 lbs. If sending more than one
box, please mark the outside of box with Box 1 of # being sent and so on. The purchase
order and payment must be enclosed in Box 1. To confirm receipt of the entries at the
Catholic Press Association office, please use your carrier’s online tracking service.
ENTRY FEE: $36.00 fee general publisher members/$46.00 fee for other CPA
Members/$76.00 for Non‐Members per entry, per category. Payment can be submitted
online or via check. If paying by check, your purchase order and payment should be
enclosed with all submissions to ensure that your entries are credited correctly.
DEADLINE: The entry submission deadline is FRIDAY, FEBRUARY 8, 2013
